Registration for the 2021-2022 school year
Due to COVID-19, we will continue to help families remotely register for the 2021-2022 school year. In the coming days, all incoming 7th graders will be receiving registration information from their elementary school Principal. If you do not get an email from your elementary school, please click on this link to review the registration information.
If you are new to the community and Bonita Vista Middle School is your home school, you will begin by completing the OLR (Online Registration) Household form. Fill it out using a PDF editor or print it and complete it by hand, and then send to the following email address: bvmregistration@sweetwaterschools.org
Your student will not be able to attend any classes for Fall 2021 until all requirements have been completed. See the Frequently Asked Questions below for information on how to proceed:

Registration information for Parents and Guardians During School Closures
- When can I begin to register for the 2021-2022 school year?
- The On-Online Registration (OLR) system will not be open until March 15, 2021. If you are not from one of our feeder schools and want to get a head start on the process, please download this form to your computer, fill it out using a PDF editor or print it and complete it by hand, and then send to the following email address: bvmregistration@sweetwaterschools.org
- If you attend one of our feeder elementary schools, your information will be provided to our school district by the CVESD so we can add your student to our system. Once you are in our system, you will receive a letter from SUHSD with a username and activation code so you can begin the OLR process. Letters will start to go out after March 15, 2021. If you do not receive a letter in the mail or in your email, request your activation code by filling out this form [link]. You will receive a response within 24 hours (Monday-Friday).
- Is Bonita Vista Middle (BVM) the school my student should be attending?
- To register your student at BVM you must live within our school boundaries. You can determine this by clicking on this link to access the My School Locator website or you can view the district boundaries map here [link]
- If you do not live within the BVM school zone you must apply for a transfer with Student Support Services (619) 691-5564.
- When can I register my student for the 2021-2022 school year?
- OnLine Registration (OLR) will take place from March 15 – July 2, 2021.
- Your student must first be currently enrolled prior to submitting an OLR application.

- What if I am new to the school or not currently enrolled?

- Is my student automatically enrolled once I submit the OLR Household form?
- No. Once we receive the form we enter your student into the system and then we will email you a username and activation code with instructions on how to proceed next.
- I have my letter with the username and activation code, now what?
- Once you have your activation code, you will need to go to the Campus Portal and create an account. Follow these instructions to create your account and begin the On-Line Registration (OLR) process.
- Are there any other forms I will need to submit after I complete the OLR process?
- Yes. First, please download the Affidavit Verification of Residency, fill out, and submit, with proof of residency, via email to the registrar Ms. Toni Lehotsky: bvmregistration@sweetwaterschools.org

- Second, you will need to submit copies of verification of residency documents listed on the form such as a utility bill, lease agreement/rental contract, or any other acceptable documentation on this list.
- Since we are not allowed to visit the school to drop off any residency verification paperwork, how will we submit it?

- What if I don’t have my username or activation code?
- Fill out this form to request another copy of the letter with your username and activation code via email within 24 hours (Monday – Friday).
- I have an account for the Campus Portal, but I can’t remember my password. What do I do?
- Click on “forgot your password?” This will allow you to enter your e-mail address, click send reminder and you will get an email with your password. This may take a few minutes to arrive in your email.
- If no email is received, fill out this form to get the information emailed to you within 24 hours (Monday – Friday).
- What else will I need to take care of before my student can attend Bonita Vista Middle?
- Meet the immunization requirements. This step will need to be verified by the school nurse.
- Complete the Course Selection form prior to getting your classes assigned (the Registrar or Counselor will send those to you once you are officially enrolled).

- Current Immunization Record (See link for all required immunizations that must be present on provided documentation)
*Proof of Residency Pay Stub, Utility Bill: Gas & Electric, Water, Cable TV, Internet, Trash or Sewer Verification of Social Services, Correspondence from Government Agency. Lease Agreement/Rental Contract with Letterhead of the complex, business card from apartment manager and a current rental receipt. (must be within 60 days of enrollment date)
JOINT RESIDENCY REQUIREMENTS
Parent/caregiver must provide all three:
- California driver’s license, CA Identification with current address
- Current Bank statement OR verification of social services with current address
- Current utility bill showing current address( SDGE, water, cable, internet, sewer, trash, pay stub, mortgage, correspondence from government agency)
